I, the undersigned parent or guardian, submit that my son or daughter is physically fit to participate in strenuous athletic activity and is able to participate in the Built Ford Tough Youth Football Camp hosted by Max Starks. In Consideration of my child/children entry being accepted, I intend to be legally bound, and do hereby for myself, my heirs, executors, waive and release all rights and claims for damages which may or hereinafter accrue to me against Max Starks, The Max Starks Charitable Fund at the Pittsburgh Foundation, Lake Highland Preparatory School, Ford Motor Company, Southern Ford Dealers, Connect Ford, Signature Events & Marketing LLC, Event Xperts, volunteers, sponsors which I'm entering, subsidiary divisions, officers, agents, directors, representatives, successors, administrators, employees, trustees and partners for any and all damages of injuries which may be sustained and/or suffered by my child/children in connection with entry or participation in the Built Ford Tough Youth Football Camp hosted by Max Starks.

If my child suffers an injury or illness, I authorize officials of the camp to use their discretion to have my child transported to a medical facility and I take full responsibility for these actions. I attest and certify that my child is physically fit and is sufficiently conditioned for this camp. I hereby grant full permission to videotape, photograph and record my child/children activities in the Youth football camp for any purpose.
